• Individual will receive a card from us with a sticker chosen at random. All nominations will be sent out on Friday's. If nomination is placed on a Friday, they will be sent the following week.

  
  

• Address can be physical, PO Box, or clinic address. Wherever the individual can receive mail.

• They can be sent by you or anonymously. Be sure to select which option you prefer.

  
  

• Currently only open to U.S. & Canada residents, but may change if we get donations to help with cost.
